Home Health VisitSouth Coast Medical Center Home Care is a licensed, Medicare-certified agency which provides high standards of health services for individuals in the comfort of their own home. Home care therapies are designed to meet each patient's needs. Services include:

  • Physician ordered care, including skilled nursing, physical therapy, occupational therapy, speech therapy, certified home health aide services and medical social service visits.

  • A professional team of RNs and LVNs to manage the patient's care.

  • An interdisciplinary team of health professionals who confer to ensure appropriate utilization and coordination of services.

  • 7 days a week, 24 hours a day, on-call consultation coverage available. South Coast Medical Center Home Care clinical staff members utilize pagers for efficient and courteous response time when after hour visits may be required.

  • Referral coordination to community resource providers for continuous care.

  • Terminal care and hospice philosophy-based care that allows continuity in nursing care when a transfer to hospice services is requested.

The clinical professionals with South Coast Medical Center Home Care are dedicated to providing expert care in a compassionate manner. An encounter with one, several, or all of the following South Coast Medical Center Home Care team members may occur during recovery and home visits.

Home Care Team

NURSES

Experienced registered nurses or licensed vocational nurses provide skilled nursing care such as wound care, catheter changes, and observation for symptoms of complications. The nursing staff also provides patient instruction in medication management, diet counseling and self-care techniques.

INFUSION THERAPY NURSES

IV therapies are performed by specially-trained registered nurses. Therapies include antibiotics, TPN and pain management.

PHYSICAL THERAPISTS

Registered physical therapists provide treatment and instructions to improve movement by increasing strength and mobility.

OCCUPATIONAL THERAPISTS

Experienced occupational therapists provide treatment and instruction to improve ability to perform daily tasks such as dressing, eating and other activities of daily living.

SPEECH THERAPISTS

These qualified therapists provide treatment to improve communication skills and enhance self-esteem.

MEDICAL SOCIAL WORKERS

Medical social workers assess significant social and emotional factors which may be related to health problems. They also provide information on community resources available to assist the patient and family in coping with changes brought along by the illness.

CERTIFIED HOME HEALTH AIDES

Certified home health aides assist patients with personal care such as bathing, skin care, hair shampooing, meal preparation and other daily personal-care needs.

REGISTERED DIETITIANS

Registered dietitians are available for telephone consultations to provide nutritional guidance and evaluate dietary needs, as determined by the patient's physician and nurse.

Health Plans and Appointments

Most health plans pay for intermittent skilled care. A reimbursement specialist will verify health plan coverage and bill directly. The most common payors of care are:

  • HMOs
  • PPOs
  • Medicare
  • Private Health Plans
  • Workers Compensation

Routine patient visits are scheduled according to each patient's personalized plan of care and therapy needs. Nurses are available on an on-call basis — day or night — through a 24-hour answering service. The general office hours are 8:00 a.m. to 5:00 p.m., Monday to Friday.
